The purpose of this Notice of Funding Opportunity (NOFO) is to provide funding to states for the implementation of section 1945 of the Social Security Act (the Act), entitled State Option to Provide Coordinated Care Through A Health Home For Individuals With Chronic Conditions. The purpose of this Notice of Funding Opportunity (NOFO) “Health Home for Individuals with Chronic Conditions” is to award funding to States that would like to receive support in planning
their health home State Plan Amendments (SPAs), in accordance with section 1945(c)(3) of the
Act. CMS will use title XIX funding to support state health home planning efforts.
