The Sexual Risk Avoidance (SR26) Continuation Solicitation is administered by the Ohio Department of Health (ODH), Bureau of Maternal, Child, and Family Health. Funded through the U.S. Department of Health and Human Services' Family and Youth Services Bureau, the grant supports educational and community-based efforts to reduce sexual risk behaviors among youth. This continuation grant is strictly for agencies already receiving funds through Ohio's Sexual Risk Avoidance program and aims to sustain and enhance existing services in defined regional target counties. The grant operates on a 100% deliverables-based reimbursement model, meaning funding is disbursed based on achievement of specific, measurable outcomes. These deliverables include youth education or direct services, sustainability and community outreach activities, and program evaluation. Each of the four regional awards will receive approximately $500,000, totaling $2 million. Only The Ridge Project and Relationships Under Construction are eligible to apply, with assignments by region clearly specified. The grant period for this continuation cycle spans from October 1, 2025, to September 30, 2026, as part of a broader project period running from October 1, 2021, to September 30, 2026. To qualify, applicants must submit a complete electronic application through the Grants Management Information System (GMIS) by 4:00 p.m. on Tuesday, May 27, 2025. Applications received after the deadline or submitted through fax, mail, or in-person delivery will not be considered. Only electronic submissions are accepted. Required documents include a program progress report, updated program narrative, SMART objectives and work plan, detailed budget and narrative, health disparity documentation, and compliance assurances. Additional documents such as liability insurance for nonprofits and Civil Rights Review Questionnaires must also be submitted. The evaluation process includes a detailed scoring rubric that covers aspects like clarity and completeness of the proposal, program narrative and progress, SMART objectives, adherence to formatting, and budget clarity. Applicants are expected to align their goals with ODH’s strategic priorities, especially in addressing health disparities and promoting cultural and linguistic competence (CLAS). Programs must also demonstrate compliance with federal and state grant assurance standards and show capacity to front costs for up to eight weeks before reimbursement. Reporting requirements for successful applicants include mid-year and annual program reports as well as monthly and quarterly financial expenditure reports. These must be submitted via GMIS and align with ODH’s policies. The deliverables-based funding is broken down as follows: up to 80% for youth education services, 15% for sustainability and outreach, and at least 5% for evaluation. Payment is contingent upon submission of acceptable performance and expenditure reports. ODH reserves the right to withhold funds for non-compliance or prohibited uses.
