Grant Description

The Northern New Mexico Health Grants (NNMHGG) is a collaborative funding initiative administered by the Con Alma Health Foundation (CAHF) in partnership with the Hospital Auxiliary of Los Alamos Medical Center. These grants aim to support unmet health care needs in specific communities traditionally served by the Los Alamos Medical Center, specifically within Los Alamos County, Rio Arriba County, and northern Santa Fe County. This initiative is part of the broader mission of both organizations to foster health equity and improve healthcare access for underserved populations. The primary purpose of the NNMHGG is to fund projects that address pressing health issues affecting marginalized and underserved populations in the designated geographic areas. These projects should work towards strengthening safety-net systems, engaging in advocacy, and promoting policies that are driven by and for the community. Additionally, the initiative focuses on expanding health equity by eliminating disparities in healthcare access and outcomes, enhancing opportunities for vulnerable populations, and tackling social determinants of health as outlined by the CDC’s Healthy People 2030 framework. Applicants may request funding amounts of $10,000, $15,000, or $20,000 for a one-year project implementation period spanning from January 1, 2026, to December 31, 2026. The total anticipated funding for this grant cycle is $200,000. Eligible applicants include nonprofit organizations with tax-exempt 501(c)(3) status or those applying through a fiscal sponsor with such designation. Organizations currently or previously funded by CAHF or NNMHGG are also eligible. Work must exclusively benefit the residents within the target counties and regions. The application period opens on May 2, 2025, and closes at 11:59 PM on May 30, 2025. Notification of award decisions will be made in November 2025, and funds will be disbursed by December 2025. Required documentation includes a detailed project budget, the organization’s operating budget for the current fiscal year, and, if applicable, a tribal support letter. Incomplete applications or those lacking necessary documentation by the extended deadline will not be considered. Proposals will be evaluated for completeness, eligibility, timeliness, and alignment with NNMHGG’s focus areas. Funding may cover a range of expenses including staffing, professional services, operations, and limited equipment purchases (up to $5,000 per item). However, grants cannot be used for political activities, support to individuals, fundraising, debt liquidation, or capital projects. For support with the grant portal or application process, applicants can contact [email protected].
