Grant Description

The Money Follows the Person (MFP) Transition Center program, administered by the New York State Department of Health’s Center for Home and Community Based Services through the Division of Community Integration and Alzheimer’s Disease, is designed to support Medicaid recipients transitioning from institutional settings back into community living environments. This initiative is part of a broader effort to rebalance long-term services and supports in New York State by prioritizing home and community-based care over institutionalization. The MFP program operates under the mandates of federal law, including Title II of the Americans with Disabilities Act and the Olmstead decision, and supports transitions for elderly individuals and those with physical, developmental disabilities, or traumatic brain injuries. This Request for Applications (RFA 20599) aims to select a single not-for-profit organization to manage a statewide Transition Center infrastructure. The selected entity will be responsible for delivering core services such as transition assistance, education and outreach, peer and informal support, and ancillary transition aid. The awarded organization must maintain a statewide presence and be capable of directly providing or subcontracting services across all 62 New York counties. A total of $41,000,000 in funding is allocated over a five-year period beginning July 1, 2026, with an annual award of approximately $8.2 million. Eligibility is restricted to New York State-based not-for-profit organizations that are prequalified through the Statewide Financial System (SFS) and hold IRS 501(c)(3) status. These organizations must demonstrate an ability to meet the program’s objectives, including infrastructure development, stakeholder engagement, outreach, and rigorous data collection and reporting. Only one application per agency is allowed, and any duplicate submissions will be rejected. Applications must be submitted through the New York State SFS Vendor Portal. Key application components include a comprehensive work plan, detailed budget, program summary, organizational capacity narrative, and optional but recommended subcontractor commitment letters. Attachments such as the Application Cover Sheet, Vendor Responsibility Attestation, and M/WBE documentation are required. The Department of Health strongly recommends early submission to mitigate technical issues, and the grant process includes both a question phase and technical support resources. The application timeline is as follows: the RFA was released on May 5, 2025; questions are due by May 23, 2025; responses will be posted by June 5, 2025; and completed applications are due by June 20, 2025 at 4:00 PM ET. Evaluation will be based on a scoring system totaling 100 points, with minimum passing scores set at 70. Questions about the RFA should be directed to Stacey Agnello at MFP@health.ny.gov. The selected grantee must adhere to rigorous payment, reporting, procurement, and conflict-of-interest requirements.
