Grant Description

The Ohio Department of Health (ODH), through its Bureau of Health Improvement and Wellness and the Violence and Injury Prevention Section, is offering continuation funding under the Injury Prevention-Falls Among Older Adults (IF26) program. This program aims to reduce fall-related injuries and deaths among older adults in Ohio. The funding period for this cycle spans from October 1, 2025, to September 30, 2026, with continuation anticipated through September 2028. Applications are due electronically by June 9, 2025, at 4:00 p.m., and must be submitted via the ODH Grants Management Information System (GMISP). Applications submitted by fax, mail, or hand delivery will not be accepted. The IF26 program is funded by the federal Preventive Health and Health Services Block Grant (PHHSBG) and allows for up to six awards with a total funding pool of $642,000. Each eligible agency may request up to $107,000, with a minimum request of $30,000. Two optional supplemental funding opportunities are also available: one for Union County Health Department to manage the Ohio Older Adult Fall Prevention Coalition (up to $56,000) and another for planning a statewide virtual Ohio Injury Prevention Conference (up to $24,000). Eligible applicants must not owe funds to ODH, must not be referred to the Ohio Attorney Generalโ€™s Office, must meet federal certification standards, and must have proper cybersecurity and whistleblower protections in place. Nonprofits must provide documentation of liability coverage and nonprofit status. All applicants are required to have current SAM.gov registration and complete FFATA forms. The application package includes a program narrative (not required this year), updated SMART objectives and work plan, a program progress report, and a detailed budget narrative. All documentation must be prepared according to strict formatting guidelines. Funding is deliverables-based and supports activities such as coalition meetings, strategic planning, participation in state coalitions, Fall Prevention Awareness Week events, professional development, capacity evaluations, and implementation of up to four community-based policy, systems, and environmental change (PSEC) strategies. Agencies must also designate one full-time staff member solely dedicated to administering the grant. Evaluation of applications is based on scoring criteria outlined in the Application Review Form. Reporting is a critical component of the grant, with quarterly program and financial reports due throughout the funding period. Final expenditure reports must be submitted by November 5, 2026. All reports are submitted through GMISP, and adherence to deadlines is crucial to continued funding eligibility.
