Grant Description

The Ohio Department of Health (ODH), through its Bureau of Health Improvement and Wellness, is offering continuation funding under the WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across the Nation) program for the budget period from October 1, 2025, to September 30, 2026. This opportunity, funded with federal dollars, is part of a longer project period extending from April 1, 2024, through September 30, 2028. The WISEWOMAN program aims to reduce cardiovascular disease risk factors among under- and uninsured women aged 35-64 through screenings, case management, and connections to social services. The initiative aligns with the Ohio Department of Health’s broader commitment to reducing health disparities and achieving health equity. Eligible applicants for this grant must be currently funded Ohio WISEWOMAN Southeast Regional Enrollment Agencies that are in good financial standing with ODH, have not been certified to the Attorney General’s office, and have submitted their applications and all required attachments by the stated deadline. Applications must be submitted electronically through the GMIS (Grants Management Information System) and must follow strict formatting and documentation guidelines. New staff members requiring access to GMIS must complete mandatory training provided by ODH. The grant offers one award not to exceed $92,000 and not less than $30,000, exclusive of matching requirements. Funding may support personnel, training, travel, supplies, equipment over $1,000 (with proper justification), and contracts that align with the program’s goals. Matching is not required. However, the budget must follow applicable federal cost principles and OGAPP (ODH Grants Administration Policies and Procedures). Certain costs are unallowable, including lobbying, entertainment, inpatient services, and promotional items. Applicants must provide updates on personnel, partnerships, workplans, SMART-IE objectives, and efforts to reduce health disparities. Program narratives must outline successes, challenges, and modifications to reach targets. Additionally, documentation on health equity activities and specific deliverables is mandatory. The workplan must include strategies for cardiovascular disease screening, case management using the Med-IT database, and building bidirectional referral systems with providers and community partners. Applications are due by 4:00 p.m. on Monday, June 2, 2025. Monthly program and expenditure reports must be submitted in GMIS by the 10th of each month, with a final expenditure report due by October 5, 2026.
