Grant Description

The Trumbull Memorial Health Foundation is committed to enhancing the health and well-being of residents across Trumbull County, Ohio. The foundation acknowledges that health disparities exist across various demographic and geographic lines, and its grantmaking is structured to address those differences with a focus on equity. This includes consideration for factors such as race, gender identity, disability, and urban or rural locality. Grants awarded by the foundation vary in size and scope to reflect the complexity and depth of barriers faced by different communities. The foundation’s funding priorities for 2024 are centered on the strategies outlined in the 2023–2025 Trumbull County Community Health Improvement Plan (CHIP). These include addressing mental health and substance abuse, particularly through community coalitions and expansion of evidence-based practices. Other key areas include improving access to care, with an emphasis on culturally congruent services and data-driven policy advocacy, and improving community conditions and safety through better housing, transit, greenspace, and food access. Additional funding considerations are made for organizations focused on specific health-related issues such as cancer care, hospice services, diabetes education and prevention, and orthopedic support and education. The foundation encourages applications that include leadership by individuals with lived experience, clear success metrics, use of both qualitative and quantitative data, and efforts that tackle social and economic determinants of health. Grants are evaluated on their ability to leverage additional funding and their engagement with cross-sector partners including those in healthcare, education, and public health. Strong preference is given to applicants aligned with the Healthy Community Partnership, a regional collaborative focused on implementing CHIP priorities. Applications are accepted on a rolling basis, allowing applicants to submit proposals at any time. However, completed applications are reviewed only after one of the three annual deadlines: April 15, July 15, or October 15. Funding decisions are typically made within three months of each grant cycle deadline. Applicants are advised to ensure alignment with CHIP strategies and engage appropriate community and sectoral partners for stronger applications.
