2025 Fall Proactive Grant Program

The Superior Health Foundation (SHF) is currently offering its Fall Proactive Grants as part of its broader Spring and Fall Proactive Grants Program. This funding initiative is aimed at supporting health-centered projects across Michiganโs Upper Peninsula. The program's purpose is to drive improvements in health outcomes by funding initiatives that address core regional health challenges. SHF places a particular emphasis on projects that improve accessibility, expand behavioral health services, offer chronic disease prevention and intervention, enhance healthy food access, support maternal-infant health, assist older adults and caregivers, expand oral health access and education, and advance workforce development and training. Eligible applicants include any tax-exempt organizations based in or actively working in the Upper Peninsula of Michigan. The program gives preference to grassroots organizations, neighborhood groups, tribal communities, and small- to medium-sized nonprofits. While priority is given to the listed health focus areas, SHF is open to other types of health-related projects that show clear benefits to the community. Applicants must begin by scheduling a consultation with SHF or sending a one-pager detailing their project idea. Following that, they must log into the SHF grants portal to complete their application. Required materials include a detailed project narrative outlining the target population, need, proposed solution, and intended outcomes, as well as a work plan, evaluation plan, budget and budget narrative, and at least two letters of support. The application will be evaluated based on the number of people served, the level of community engagement, innovation in addressing health needs, potential to improve health outcomes, and collaboration with other entities. The grant awards range from $5,000 to $75,000, with a total of up to $750,000 allocated for the year 2025. Organizations may apply for either one or two years of funding. Capital projects, loans, litigation, lobbying, and discriminatory organizations are not eligible for funding. Successful applicants will be expected to participate in regular check-ins or submit progress and budget updates. The Fall Proactive Grants opened for applications on May 7, 2025, and the submission deadline is July 9, 2025. Award announcements will be made in September 2025.
