2025-2026 Community Wellbeing Grant

The Community Wellbeing (CWB) Grant is an initiative offered by Tri-City Mental Health Services in partnership with local communities to support mental and emotional wellness. The program provides funding and technical assistance to projects based in Claremont, La Verne, and Pomona, California. This initiative recognizes the essential role communities play in fostering overall wellness and aims to support sustainable efforts that address local mental health needs, especially among children and transition-age youth aged 0 to 25. The CWB program offers monetary grants of up to $10,000 to fund community-led projects that enhance mental and emotional wellbeing. In addition to funding, recipients receive technical assistance, ongoing support from Tri-City staff, and access to resources such as trauma-focused trainings and mental health awareness tools. Projects are also given tools to evaluate their impact, build community connections, and share knowledge with other groups working toward similar goals. To be eligible for the grant, applicants must either be youth-led groups or fund projects directly benefiting youth aged 0 to 25 within the cities of Claremont, La Verne, or Pomona. All applicants must be connected to a nonprofit organization with 501(c)(3) status, which will serve as their fiscal sponsor. Applications are accepted only from communities that attended the mandatory Bidder’s Conference held in February 2025. Applications for the 2025-26 fiscal year must be submitted by 12:00 PM on Tuesday, April 1, 2025. Submissions should include a completed application form, a copy of the 501(c)(3) documentation (either from the applicant or their fiscal sponsor), and must be signed by community leadership. Electronic signatures are accepted, and the complete application should be submitted by email to Daisy Martinez, Community Capacity Organizer, at dmartinez@tricitymhs.org. Evaluation of applications will likely focus on the project’s sustainability, community relevance, and potential impact on youth mental wellbeing. Successful applicants will be able to demonstrate clear goals, alignment with community needs, and readiness to engage in ongoing support and evaluation activities provided by Tri-City Mental Health.
