Quest Diagnostics Foundation is providing a funding opportunity through its Priority Communities initiative, specifically targeting communities within Miami-Dade County, Florida. Eligible applicants must be U.S.-based tax-exempt entities, public charities, or governmental organizations, including institutions like public schools, universities, and health departments. National nonprofits may apply if they partner with community-based organizations in Miami-Dade County. For-profit entities, individuals, and organizations operating outside of the designated geography are not eligible. In addition, the Foundation will not support capital campaigns, unrestricted general support, or events not aligned with long-term health objectives. The application process consists of a two-stage format: short proposals followed by full proposals for selected applicants. The application portal opened on April 24, 2025, with short proposals due by June 6, 2025. Applicants invited to submit a full proposal will have eight weeks from their invitation to complete the submission. Full proposals will undergo detailed evaluation, and award status notifications will be delivered within eight weeks of submission. The short proposal requires details such as community demographics, program goals, projected reach, use of Quest Diagnostics’ testing resources, funding estimate, and program timeline. Full proposals are evaluated based on program overview and implementation (40%), operational capacity (30%), evaluation and sustainability plans (20%), and budget details (10%). Required attachments include IRS determination letters, audited financial statements, logic models, and budget templates.
