The Arizona Department of Health Services (ADHS), through its Bureau of Chronic Disease and Health Promotion (BCDHP), is seeking proposals for the Community Health Worker (CHW) Community-Clinical Linkage Partnership/Program. The goal of this program is to enhance community-clinical linkages to reduce the impact of chronic diseases such as heart disease, stroke, lung disease, cancer, diabetes, and asthma in Arizona. These chronic conditions are among the most common, costly, and preventable health issues in the state. This grant opportunity is designed to foster collaborations between community health workers, health care providers, and community-based organizations to improve access to care and patient outcomes. This grant supports efforts that leverage community-clinical partnerships to improve the delivery of chronic disease care, with a focus on reducing health disparities among underserved populations. The program encourages local governments, academic institutions, tribal health providers, and nonprofit organizations to apply, particularly those with the capacity to integrate community health workers into care models that bridge the gap between community services and clinical care.
