Grant Description

The funding opportunity HRSA-25-017 (Service Area Competition - SAC) aims to ensure the continuity of affordable, accessible, and high-quality primary health care services for underserved communities. The total anticipated funding is approximately $270 million, and up to 81 awards will be made for organizations to serve designated service areas. Applicants can apply for competing continuation, competing supplement, or new awards. The performance period is from June 1, 2025, to May 31, 2028. Eligible applicants must be private non-profits or public agencies, including tribal and community-based organizations. They must provide comprehensive primary health care services to the underserved population in the designated service area without regard to a patient’s ability to pay. Applicants are required to serve at least 75% of the patient target in the service area listed in the FY 2025 Service Area Announcement Table (SAAT). Funding reductions will be applied if they propose to serve fewer patients. No cost-sharing or matching is required. Applications are submitted in two phases: an initial application through Grants.gov by October 29, 2024, and supplemental information through the HRSA Electronic Handbooks by December 2, 2024. Important evaluation criteria include need, response, collaboration, resources, governance, and patient outcomes. Successful applicants will demonstrate comprehensive service plans, community collaboration, and financial and operational readiness to maintain and expand primary care services in their area.
