Approximately half of the awarded projects are expected to come from rural communities in Colorado. Eligible applicants must be 501(c)(3) nonprofit organizations or have a 501(c)(3) fiscal sponsor. Government agencies are also eligible if they collaborate with a nonprofit and demonstrate community impact on senior wellness through physical health, nutrition, or chronic condition management. All applicants must be located in or serve populations in Colorado. Funding is not available for projects focused on advocacy, capital campaigns, for-profit entities, planning grants, student scholarships, insurance payments, or medication expenses.
