Eligible applicants include agencies that are current subrecipients in good standing with ODH, have no outstanding debts, and are not under legal or financial sanction from the Attorney General’s office. The program mandates that all application components be submitted electronically through the Grants Management Information System (GMIS) by 4:00 p.m. on May 19, 2025. Hand-delivered, mailed, or faxed applications will not be accepted. Submission constitutes acknowledgment of compliance with ODH policies, federal and state regulations, and the original competitive solicitation. Required components include a program narrative detailing updates and progress, a revised SMART objectives workplan, documentation of health disparity reduction strategies, a detailed budget justification, and additional attachments including a CLAS strategic plan and evidence of insurance and nonprofit status where applicable. Five key deliverables are defined for continuation funding: maintaining an Adolescent Health Coordinator position, conducting trauma-informed care training, updating and implementing a community health training workplan, facilitating a community advisory committee, and providing comprehensive monthly and quarterly reporting. Emphasis is placed on using data dashboards like Ohio Health Improvement Zones and Youth Wellbeing metrics to target community needs effectively. The grant explicitly prohibits expenditures on lobbying, entertainment, and other non-program-related activities and requires adherence to federal cost principles.
