The Chatham County Hospital Authority (CCHA) Trust announces a $1.2M grant funding opportunity and invites nonprofit and for-profit organizations providing preventative health care services to the indigent population in Chatham County, Georgia, to apply for funding through a competitive grantmaking process. While United Way will facilitate the grant process, CCHA will retain full decision-making authority.   The purpose of the CCHA Trust Grant is to provide preventative health care services to the indigent population of Chatham County, Georgia.   Areas of Focus Must address at least one of CCHA’s focus areas: Behavioral and mental health services Health screenings Dental hygiene care Specialty care services (e.g. diabetes) Patient advocacy and navigational resources Funding Information Individual grant awards will not exceed $250,000. Eligibility Criteria Must serve indigent Chatham County, Georgia residents “Indigent” is defined, for this purpose, as households at or below 138% of the Federal Poverty Line in alignment with the U.S. Department of Health and Human Services. The current income level for this funding cycle is equal to $20,783 per person or $43,056 for a family of four. Must provide documentation to certify client household income and location. Organization/entity must be a 501(c)(3) nonprofit or for-profit business with accompanying business licensing and certifications. Must provide proof of insurance (e.g. nonprofit liability insurance or for-profit malpractice insurance certificate). Organization/entity complies with the USA Patriot Act and all federal and state laws concerning discrimination based on age, race, religion, gender or gender identity, sexual orientation, economic status, disability, cultural heritage or national origin. For-profit entities must provide: Most recent tax returns Last completed income statement (profit and loss) Balance sheet Last completed audit, compilation, or review  Non-profit entities must provide: Most recent tax returns Last completed income statement (profit and loss) Balance sheet Last completed audit, compilation, or review Organization/entity complies with all CCHA Trust eligibility requirements and bylaws. Organization/entity has a conflict-of-interest policy. Grant funds cannot be used for capital campaigns or fundraising events.
