The Harm Reduction Community Linkages Project is to support capacity building at harm reduction organizations statewide to expand their client base and develop more comprehensive linkages to prevention, treatment, and harm reduction services for individuals with opioid use disorder (OUD), including prescription opioids as well as illicit drugs such as heroin. This project is funded through the Centers for Disease Control and Prevention (CDC) Overdose Data to Action – States (OD2A-S) Cooperative Agreement and Illinois General Revenue Funds. Organizations selected for this funding will need to demonstrate success in engaging individuals with opioid use disorder (OUD), including prescription opioids as well as illicit drugs such as heroin; have experience in linking individuals to opioid use prevention, OUD treatment and injection harm reduction services through community referral networks; and show capacity to cover large geographic regions in Illinois focusing on communities that demonstrate a high burden of opioid overdose. These funds will be used to strengthen partnerships between injection harm reduction community stakeholders and providers of OUD treatment and other needed social services to improve local coordination, connect more individuals to the appropriate support services and treatment for OUD, and provide case management as needed among persons who use opioids.

Utilizing CMP funds from the federal CMS, an organization, agency, or entity would provide training to long-term care facility staff regarding ensuring proper resident hygiene, nutrition, and promoting social interation. In an effort to support the maintenance of cognitive and mobility skills, the Illinois Department of Public Health, Office of Healthcare Regulation (OHCR) announces an opportunity for certified facilities to submit applications to implementation changes to increase the quality of life within their facility. This grant seeks to provide facility staff training in the areas of; (1) showering and personal care, (2) nutrition, choking, and dietary issues, (3) social interaction / social isolation, and (4) showering and personal care. By addressing each topic individually for two hours bi-annually, the overall quality of life within Long Term Care facilities will increase.

Utilizing CMP funds from the federal CMS, facilities will apply to obtain this grant to provide the Head-to-Toe Infection Prevention training in long-term care facilities. In an effort to support the maintenance of cognitive and mobility skills, the Illinois Department of Public Health, Office of Healthcare Regulation (OHCR) announces an opportunity for certified facilities to submit applications to implementation changes to increase the quality of life within their facility. This grant seeks to provide facility staff training in the areas of; (1) wound treatment and care, (2) skin & oral care, (3) urinary care, and (4) adapting care to fit each resident. The grantee will utilize the Head-To-Toe Infection Prevention provided by IDPH which, by addressing each of these topics, the overall quality of life within Long Term Care facilities will increase.

Utilizing CMP funds from the federal CMS, facilities will submit proposals for various quick improvement projects within their facility. In an effort to support the maintenance of cognitive and mobility skills, the Illinois Department of Public Health, Office of Healthcare Regulation (OHCR) announces an opportunity for certified facilities to submit applications for projects that must provide the residents of long term care facilities with a program or project deliverable that meaningfully improves the quality of life or quality of care for residents in the facility. These projects should deliver direct improvements to quality of life, direct improvements to quality of care, alternatives to pharmacological interventions, alternative behavioral management, training programs and implementation.

In an effort to support the maintenance of cognitive and mobility skills, the Illinois Department of Public Health, Office of Healthcare Regulation (OHCR) announces an opportunity for certified facilities to submit applications for projects that must provide the residents of long term care facilities with a program or project deliverable that meaningfully improves the quality of life or quality of care for residents in the facility. These projects should deliver direct improvements to quality of life, direct improvements to quality of care, alternatives to pharmacological interventions, alternative behavioral management, training programs and implementation.

The Illinois Department of Public Health recognizes the need to improve immunization efforts across the state, and to this end, has established the Immunize Illinois Coalition Grant. The grant will provide initial funding to establish a single statewide coalition. The new coalition will bring together various immunization stakeholders with the goal with the primary goal of creating a collaborative environment where resources, expertise, and strategies can be pooled to address the complex challenges hindering optimal immunization coverage. The grant provides financial support for essential elements such as coalition meetings, additional staffing, services, tools, and other relevant items crucial to the establishment and sustainability of networks of organizations committed to advancing public health through improved immunization rates.

The New York State Department of Health, Office of Primary Care and Health Systems Management, Bureau of Emergency Medical Services and Trauma Systems is issuing the County Emergency Medical Services Support Grant. This grant aims to support county-level EMS systems in New York State. Eligible applicants are counties within New York State. Applications must be submitted online via the Statewide Financial System by June 26, 2024.

This funding opportunity provides financial support to licensed pregnancy centers that offer medical and counseling services related to pregnancy, ensuring they operate without deceptive practices.

The Illinois legislature created the Hospital and Healthcare Transformation Capital Investment Grant Program at 20 ILCS 3105/20i. Pursuant to that legislation, the Department of Healthcare and Family Services (HFS) and the Capital Development Board (CDB) published joint rules governing the program. The rules are found at 71 Ill. Admin. Code Part 44. This NOFO is intended to implement the program pursuant to the statute and rules. The Illinois Healthcare Transformation Program is authorized at 305 ILCS 5/14-12 (d-5)(2). The Illinois Healthcare Transformation Program (the “HTC Services Program”) funds initiatives that serve an underserved community based on a community needs assessment and that consist of collaboratives of providers across the care and community spectrum, including preventative care, primary care, specialty care, hospital services, mental health, and substance abuse services, as well as community-based entities that address the social determinants of health.

This award mechanism focuses on increasing implementation of evidence-based strategies to ensure that all Texans benefit from the cancer prevention knowledge that we currently have. CPRIT seeks to fund multilevel interventions to reduce cancer risk, disease burden, and cancer disparities. Modifiable risk behaviors include tobacco use, obesity, physical inactivity, unhealthy eating, alcohol use, sun exposure, HPV vaccination, Hepatitis B vaccination, and environmental/ occupational cancer exposures. Applications should also assess and address social determinants that contribute to cancer burden and disparities (e.g., cultural factors, unmet needs, access barriers). Interventions and communications should be structured to address the unique circumstances of the population to be served.

The Opioid Abatement Fund Council (OAFC) seeks to combat opioid overdoses in Texas by awarding funds to implement a statewide program that will distribute the opioid overdose reversal medication naloxone where it is needed most, and provide related training, including airway clearance techniques. The Texas Legislature created the OAFC in 2021 to ensure funds collected through lawsuits against pharmaceutical companies are used fairly and efficiently to combat the opioid crisis in Texas. The OAFC is made up of 13 appointed experts and the Texas Comptroller of Public Accounts (CPA) as the non-voting presiding officer.

This award mechanism seeks to fund projects that will facilitate the dissemination and implementation of successful CPRIT Prevent Program-funded, evidence-based cancer prevention and control interventions across Texas. The proposed project should be able to develop one or more "products" based on the results of a previously CPRIT Prevention Program-funded intervention project. The proposed project should describe and package strategies or approaches for dissemination to other partners, settings, and populations in the state.

Purpose: The California Department of Veterans Affairs (CalVet) is announcing the availability of up to $38 million in competitive grants. Grantees will focus on prevention and early intervention efforts with the end goal of increasing access to mental health services for veterans and their families. This initiative is to be known as the California Veterans Health Initiative (CVHI) Mental Health Support Grant Program (MHSGP). Description: Grantees will be asked to focus on prevention and early intervention efforts with the end goal of increasing access to mental health services for veterans and their families. Proposals funded under CVHI must be inclusive, equitable, and available to all veterans regardless of age, period of service, type of service, discharge status, or disability rating. Applicants must be able to meet the eligibility requirements of California Military and Veterans Code Section 881 to be considered for funding, and proposals must aim to expand the state’s capacity to provide mental health support to veterans and to fill gaps in mental health services. Applicants are required to select the program areas they are proposing and provide a description of the proposed activities and how the proposed activities would increase mental health access for California veterans. This NOFA allows for flexibility in program design to fit the specific needs of veterans and their families across the state, Outreach, Peer Support, Case Management, Outpatient Clinical Treatment, and Innovative Approaches will be eligible for funding and are further defined in the NOFA. The Grantee staff and leadership will be expected to maintain significant engagement with CalVet for monthly check-ins, operations assistance, reporting, and evaluation. Applicants are encouraged to ensure they are budgeting for appropriate levels of staff, time for data collection and reporting, time spent in CVHI meetings and trainings, and appropriate facility requirements (office space, group space, hardware and software equipment, supplies, overhead). Through an Application process responsive to the NOFA, awarded Grantees will provide clear and convincing evidence of adequate personnel, facility, and organizational capacity to effectively carry out their proposed project. The successful Applicant will describe how they meet minimum qualifications, including their breadth of experience working with Veterans, with special emphasis on treating veterans with mental health concerns. Eligibility Requirements Eligible Applicants: Nonprofit In addition to the minimum requirements described in NOFA applicants must be able to meet the eligibility requirements of California Military and Veterans Code Section 881 to be considered for funding, and proposals must aim to expand the state’s capacity to provide mental health support to veterans and to fill gaps in mental health services. Eligible Geographies: CalVet intends to ensure access to mental healthcare through the CVHI network by targeting 8 regions in California: North Valley, San Francisco Bay Area, Central Valley, Central Coast, Los Angeles, Inland Empire, Orange, and San Diego. Multi-regional and statewide proposals will also be considered. A regional map can be found in the NOFA. Matching Funding Requirement: Grantees will receive full funding for the first year of the grant, 75% of year one funding in the second year, and 50% of year one funding in the third year of the grant term. To compensate for the decrease in funding, grantees are required to provide a 25% match in the second year and a 50% match in the third year of the grant term to account for the reduction in funding.

This award mechanism seeks to support the delivery of evidence-based clinical services to screen for cancer and pre-cancer in underserved populations who do not have adequate access to cancer early detection interventions and health care, bringing together networks of public health and community partners to carry out programs tailored for their communities. Projects should identify cancers that cause the most burden in the community, have nationally recommended screening methods, and use evidence-based methods to screen for these cancers.

The Cancer Prevention and Research Institute of Texas (CPRIT) aims to fund the dissemination of successful cancer control interventions through its Dissemination of CPRIT-Funded Cancer Control Interventions (DI) award. This program supports the sharing and adoption of effective cancer prevention and control strategies across Texas, focusing on previously funded CPRIT projects. Eligible projects are those that have concluded within the last three years or are ongoing expansion efforts, with a funding cap of $450,000 over 36 months. The initiative stresses active dissemination strategies and the development of products with practical impact, targeting resource-limited and medically underserved areas.

The Indiana Department of Health (IDOH) is accepting grant applications to provide intranasal naloxone to first responders in rural counties. Only first responders who provide services in rural counties are eligible to receive the naloxone doses. For grant purposes, “first responders” includes (professional and volunteer) firefighters, law enforcement officers, paramedics, emergency medical technicians, or other legally organized and recognized volunteer organizations that respond to adverse opioid-related incidents. Please email the naloxone program manager if you think your agency might be considered a first responder agency and is not listed. First responder organizations that receive naloxone doses will be responsible for administering the doses and reporting their use to IDOH through the required methods.

To reduce the incidence of overweight, obesity, pre diabetes, and/or diabetes in Tennessee. A request for applications (RFA) is issued every three years. The next RFA is expected to be issued in January 2025. A letter of intent (LOI) is required for an invitation to apply. The Notice of Award is posted in late April or early May with a July 1 start date. Project Diabetes funds are distributed through a request for reimbursement process. Therefore, applicants must have the funds needed to implement the project upfront. Grantees are reimbursed monthly for their expenses.
